This notice explains how Little Smiles Dentistry may use and share your protected health information (PHI), and how we protect that information as required by the Health Insurance Portability and Accountability Act (HIPAA). We are required by law to maintain the privacy and security of your PHI, provide this notice, and follow the privacy practices described here.

HOW WE MAY USE OR SHARE YOUR INFORMATION

Treatment

To provide, coordinate, or manage your dental care and communicate with other health professionals involved in your care.

Payment

To bill you or your health plan and obtain payment for services.

Health Care Operations

To operate our practice, improve quality and safety, train staff, and manage administrative activities.

As Required or Permitted by Law

For public health and safety, suspected abuse or neglect, legal proceedings, law enforcement, workers’ compensation, and other purposes permitted or required by law.

YOUR PRIVACY RIGHTS

You may request access to or a copy of your medical record, request corrections, ask for confidential communications, request restrictions on certain uses or disclosures, request an accounting of certain disclosures, and obtain a paper copy of this notice. You may also designate a personal representative when legally authorized. You may file a complaint with Little Smiles Dentistry or the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.

OUR COMMITMENT TO PROTECT YOUR INFORMATION

We use reasonable administrative, physical, and technical safeguards to protect your PHI from unauthorized access, use, or disclosure. We will notify you as required by law if a breach occurs that compromises the privacy or security of your information. Uses or disclosures not described in this notice generally require your written authorization, and you may revoke an authorization in writing as allowed by law.
